Poor diabetic control impairs wound healing and response to infection. The patient may present with undiagnosed diabetes mellitus, with known preexisting diabetes or with complications of diabetes mellitus to the surgeon. Hyperglycemia may be the first indication of occult infection. Long standing diabetes may be associated with significant co-morbidities e.g. cardiovascular. Diabetic patient may need debridement in carbuncles, necrotizing fasciomyositis, amputation for gangrene and sometimes reconstruction surgery of blood vessels.
