Purpose: The usual method to measure change in quality of life QoL ; is to compare baselinewith follow-up measurements. However, patients may change the reference value by which they evaluate their QoL over time. This may invalidate longitudinal QoL measurements. A method that aims to measure this change in reference values is the comparison of a retrospective baseline- with a conventional baseline- measurement. Another method for evaluating change in QoL is the retrospective appraisal of the perceived direction and magnitude of change, i.e., transition questions. Our objective was to measure change in QoL according to conventional baseline- and follow-up measurements, retrospective baseline- and follow-up measurements, and retrospective transition questions, and to determine which measure of change in QoL yields strongest associations with clinical measures of change. Methods: HIV infected patients n 268 ; completed four MOS-HIV scales at week 0 baseline measurement ; and after 36 weeks follow-up measurement ; . At week 36, they additionally completed these scales as retrospective baseline measurement, and they answered four questions about change in QoL transition questions ; . Change from week 0 to week 36 in CD4-cell count, body mass index, and plasma HIV viral load served as external criterion measures of change. We compared Pearson correlation coefficients between the three measures of change in QoL and the three clinical criterion measures using an overall Chi-square test. Results: Improvement in QoL according to the retrospective baseline- follow-up measurement was significantly larger than according to conventional baseline- follow-up measurement, as patients evaluated their QoL to be worse on the retrospective- than on the prospective baseline measurement. This may indicate that patients' reference by which they evaluate their QoL over time had changed, which could invalidate prospective assessments of change. The method for measuring change incorporating a retrospective baseline measurement yielded strongest associations with clinical measures of change. As the other retrospective method, i.e., transition questions, yielded associations similar to the prospective method, this was not likely an artefact of the retrospective method in general. Background: Inflammatory reactions occurring in the brain after ischemia may contribute to secondary damage. Results: Delayed administration of minocyclin4 alone or delayed minocyclinee plus delayed mild hypothermia reduced the infarction volume significantly. However, delayed mild hypothermia alone was not protective and delayed mild hypothermia in combination with minocycline did not show any additive effect. Conclusions: These results suggest that minocycline is beneficial in focal ischemic brain injury, and the lack of the enhanced neuroprotection may be due to the brief exposure to hypothermia.

Tigecycline glycylcycline, GAR-936, Wyeth ; too, is likely to be submitted for registration in 2005; Phase III intra-abdominal sepsis and skin skin structure trials are well advanced. Like azithromycin, tigecycline is remarkable in its tissue penetration. Unlike other tetracyclines it will be formulated as an i.v. drug, not oral. The idea of using a tetracycline in severe infections will raise some eyebrows. Nevertheless the intra-abdominal trial is against imipenem and the skin skin structure trial against vancomycin + aztreonam - certainly not "straw-man" comparators! Moreover minocycline was widely used against MRSA in Japan, where vancomycin became available only relatively recently. Among newer agents, tigecycline is exceptional in extending the anti-gramnegative activity of a drug class. Specifically, it evades the acquired tetracycline efflux pumps that are prevalent in Enterobacteriaceae and Acinetobacter spp. as well as the efflux and ribosomal mechanisms of gram-positive bacteria. Pseudomonas spp. and most Proteeae remain resistant, as to other tetracyclines, owing to impermeability or to chromosomally-mediated efflux. Asides from polymyxin, tigecycline is the sole antibiotic active, in vitro, against some multiresistant Acinetobacter spp., and its clinical role here deserves urgent exploration. We are aware of two case reports of successful tigecycline use in acinetobacter infections. Wyeth are making tigecycline available for compassionate use, where appropriate, in infections caused by multi-resistant pathogens. BAL-9141, a broad-spectrum cephalosporin discovered by Roche and now being developed by Basilea has completed phase II and is moving into Phase III. Critically, its activity includes MRSA, also E. faecalis and penicillin-resistant pneumococci. Anti-MRSA -lactams, able to bind to PBP-2' have long been a Holy Grail, sought in a quest with as many disappointments. We know of no previous analogue to have advanced as far, and will watch developments with great interest.
