Surprisingly, studies have not identified a clear personality trait specific to patients with bipolar manicdepressive illness. Intuition may suggest that patients are labile, unstable, or perhaps seekers of novelty even when they are not manic or depressed. However, there is little evidence of specific personality characteristics.53 Evidence suggests that the first episode of bipolar disorder often is associated with stress in the life of the patient -- the classic stressful event may be a first love relationship.54 However, most studies agree that subsequent manic episodes often tend to be unrelated to external events in the patient's life. Pooled analysis of data from three placebo-controlled studies of escitaloprram in GAD found that discontinuation due to adverse events occurred in 8% on escitaloprzm and 4% on placebo. In addition, more patients on escitqlopram experienced nausea, ejaculation disorders, insomnia, fatigue, decreased libido, and anorgasmia. In the paroxetine GAD ; study, the overall incidence of adverse events was similar in both groups, but constipation, insomnia and sexual effects occurred more frequently in the paroxetine group. During the down titration phase of this study, some paroxetine treated patients reported dizziness 9.7% ; and paresthesia 6.6% ; , two symptoms of SSRI discontinuation syndrome, but these effects were not reported by escitalopram treated patients. Both escitalopram and paroxetine are licensed to treat the same anxiety disorders but the cost of a 28 days course of escitalopram 10mg-20mg d is 14.91-25.20, compared with 5.41-13.53 for paroxetine 20mg-50mg d. According to the NICE guideline on anxiety PD and GAD ; , when deciding on choice of medication, cost should be taken into account, where equal effectiveness is demonstrated. It adds that if one SSRI is not suitable or there is no improvement after a 12- week course, and if a further medication is appropriate, another SSRI should be offered The Scottish Medicines Consortium recently accepted escitalopram for use within NHSScotland for the treatment of GAD in situations where pharmacological therapy is appropriate. To date, there has only been one published study directly comparing escitalopram with a SSRI for treating an anxiety disorder vs paroxetine in GAD ; , which reported similar efficacy. In the one study in PD, escitalopram was associated with a reduced frequency of panic attacks compared with placebo p 0.04 ; . The SAD studies reported that escitalopram was more effective than placebo in improving anxiety scores and was associated with a statistically significantly higher response rate. Similar findings were reported in the placebo controlled GAD studies. These small numerical differences between escitalopram and comparator in changes in scores from baseline to end point on the various anxiety scales were reported to be statistically significant, but it is difficult to establish if they are of meaningful clinical significance in practice and
finasteride.

Although many clients prefer the injectable, providers also favor it and may steer clients away from long-term methods, which take longer to administer; require expendable supplies, or require special skills to administer. Although the longer-term methods give more CYP and, hence, are less work for providers in the long run, many provi ders have only an immediate, short-run view: What is the quickest way to serve all of the women in my waiting room today? From this viewpoint, providers can serve many injectable clients in the time it takes to serve one IUCD or implant client. Providers may have no gloves, disinfectant, water, or electricity. Clinics may have no trained personnel, a shortage of trained personnel, or personnel who need refresher training. Some providers have biases that stem from fears and misconceptions and that need to be addressed through updated training and facilitative supervision. Although the logistics management system smoothed out many of the erratic commodity supply situations of the 1980s, some problems still exist.6.
