Since publication of the study it is not surprising that there has been considerable reaction favourable and unfavourable ; from around the world. Such response is to be expected for a trial of ALLHAT's size and scope, especially since its results challenged some widely held beliefs. The authors have now formally responded to the most frequently asked questions regarding the ALLHAT results 2. o The simple question ALLHAT was designed to do primary outcome fatal CHD or nonfatal MI ; has been answered. As first step agents, the newer drugs ACE inhibitors, calcium channel blockers, and alpha blockers ; add no value over and above diuretics in preventing CHD or any of the other major forms of CVD. Although the study diuretic, chlortalidone, is not widely used in the UK, there is compelling evidence from other trials 9 cited by authors ; that diuretics exhibit a class effect. One study which reported a difference between hydrochlorothiazide and chlortalidone were based on a post hoc subgroup analysis in the US. The results of which are not as robust as RCTs and there were confounders which may have explained these differences other than pharmacological 3. Predefined secondary clinical outcomes included total mortality no significant difference ; and heart failure. Incident diabetes was not a pre-defined secondary outcome. There was early divergence of heart failure hospitalised ; incidence for lisinopril, amlodipine and doxazosin compared to chlortalidone. For lisinopril heart failure incidence was similar to chlortalidone between years 6 and 7 of the study. This divergence lessened at 6-12 months for all arms compared to chlortalidone probably due to the addition of second third line drugs. However the excessive risk for heart failure with doxazosin as monotherapy was not eliminated and this arm of the study was stopped early 4. In the USA prescribing rates of doxazosin have fallen substantially 5, whereas in the UK rates continue to rise. There was an excess of 36 patients in the lisinopril group and 154 patients in the amlodipine group who were hospitalised due to heart failure over the chlortalidone group over 5 years. In fact, chlortalidone was significantly superior to amlodipine in preventing heart failure, overall and for hospitalized or fatal cases. Chlortalidone was significantly superior to lisinopril in lowering BP and in preventing aggregate cardiovascular events, principally stroke, HF, angina, and coronary revascularisation. Although not a predefined secondary outcome diabetes incidence results were reported. Thiazide diuretics are associated with a small increase in serum glucose level 0.17 to 0.22 mmol L ; at least in the short term. These serum glucose differences did not translate into more cardiovascular events or higher all-cause mortality in the chlortalidone group and the chlortalidone benefits above ; were also seen in the diabetic cohort. Pelizaeus-Merzbacher disease PMD ; and a complicated form of familial spastic paraparesis SPG2 ; are X-linked development disorders of myelin formation caused by a mutation in the proteolipid protein PLP ; gene. We describe an extensive clinical and laboratory investigation of a family presenting an unusual form of PLPrelated disorder with the attempt to correlate genotype phenotype of such heterogeneous disease. Molecular analysis, neurophysiological studies, magnetic resonance imaging MRI ; and single-voxel proton spectroscopy 1H-MRS ; were performed in the patient and in other informative subjects of the family. Genetical analysis of the proband revealed a point mutation, Q234P, in the VI exon of the PLP gene. The same mutation was present in the female obligate carriers. In the proband, evoked potentials were altered and remained unchanged during the 7-year period of follow-up. MRI of the patient demonstrated patchy hyperintensities of the paraventricular white matter containing rare microcystic components which remained stable in four consecutive MRI controls. The mother and the grandmother presented a similar microcystic lesion in the left lentiform nucleus and in the left corona radiata, respectively. Both also had dystrophic pallidal calcium deposition confirmed by CT brain scan ; . The unusual genetic, MRI and clinical findings of our family confirm the wide variability of PLP-related disorders. The single point mutation in exon 6 could be related to the mild phenotype and to the corresponding MRI abnormalities, for example, doxazosin 1 mg.

The PMA is an exceptionally powerful stimulant and causes nearly 100% of all normal neutrophils to become transformed and reduce NBT to formazan deposits. Methods: The NBT test is used for the diagnosis of chronic granulomatous disease, but these are multiple factors that can affect the results of this test. For example some of the chemotherapeutic agents and antibiotics have this effect. The impaired capability of neutrophils to reduce NBT from a patient treated with chemotherapy and antibiotics was promoted our interest in evaluating the NBT reduction test of this case after periods of therapy. Results: A sequential increase of the capability of PMA-stimulated NBT test was found after periods of treatment.
