Recent work for the Exploratory Comprehensive Evaluation of Erectile Dysfunction ExCEED ; database demonstrates that in the general population of patients presenting to their urologist, ED negatively affects both general and disease-specific healthrelated quality of life HRQOL ; . 35 While this study provides insight into the detrimental affect of ED on quality of life, the cohort is somewhat selected, in that all of the patients were seen in sexual dysfunction clinics and therefore may have been more likely to be bothered by their condition and to report worse quality of life. However, population-based studies of ED in prostate cancer survivors also document that ED has a negative effect on general health. Penson, et al. 36 studied HRQOL in 2, 306 prostate cancer survivors 2 years after their diagnosis. They noted that men with ED defined as erections that were insufficient for sexual intercourse ; had significantly worse general HRQOL when compared to prostate cancer survivors who were potent. Importantly, this association remained in a multivariate analysis that controlled for 31 other potential confounding variables. Finally, this association was noted in both the physical and mental domains of general quality of life, indicating that ED has a much broader effect on quality of life than one might expect.

The study group included puerperal subjects who had their infants admitted to the neonatal intensive care unit at St. Joseph's Health Care London, in London, Ont. All mothers were mechanically expressing breast milk to be fed to their infants through a nasogastric tube. After informed consent was obtained, mothers were randomly assigned to receive either domperidone 10 mg orally 3 times daily ; or placebo for 7 days. Domperidnoe tablets were crushed and mixed with lactose. The resulting powder was placed in clear capsules, as was plain lactose powder, which acted as the placebo. Breast milk was collected using the Lactina double breast pump Medela Canada, Mississauga, Ont. ; . Each subject received an ample supply of collection bottles. These one-time use sterile containers accurately measure the volume of milk to the millilitre. The women were instructed to use a new container for each pumping and not to add milk together from 2 different pumpings. They were given recording sheets and adhesive labels to record the amount of milk collected, the date and the time. Milk volume was measured daily. A small amount of milk was retained from all patients on study day 5 for the measurement of domperidone concentrations. The mothers were instructed to record any side effects during treatment, particularly dry mouth, headache, insomnia, abdominal cramps, diarrhea, nausea and urinary retention. The record was reviewed with each woman on day 5 and on day 10 3 days after the last dose of the study medication ; , at the time of the visit for blood sampling. Three blood samples were obtained from each patient to determine serum domperidone and prolactin concentrations before the initial dose, on day 5 and on day 10. All serum and milk samples were kept frozen at 20oC for later analysis. Serum prolactin concentrations were determined using the IMx prolactin assay. The Local Ryan White Planning Council is a Board of Supervisors appointed entity charged with developing policy and planning for aids services across the LA County. Mr. Ballesteros has been an active member of the hiv Commission for more than seven years. Mr. Ballesteros has more than a 14year history in dealing with the hiv aids epidemic in Los Angeles County. As an individual who has been personally living with hiv for more than 15 years, Mr. Ballesteros understands first-hand the complications of obtaining care, treatment and support services necessary to maintain one's health.
