Daniel I Newman, M.D., N.D. 2005 VI. Energetic Neuromodulation Energetic neuromodulation techniques are perhaps the most controversial modalities for the complementary treatment of chronic pain. Undoubtedly, that is because their mechanisms of action are either obscure, or frankly contradictory to our current scientific understanding. Hence, there is resistance to accepting the potential validity of such interventions, even when empiric evidence is promising. Homeopathy is a classic case in point. This treatment model, which dates back two centuries, involves the use of substances that have been prepared in such a way that the medication termed `remedy' ; is generally serially diluted in an aqueous solution, then percussively shocked termed `succussion' ; with each dilution. Frequently, the dilution succussion cycle is repeated until the calculated concentration of the original substance is below Avogadro's number; i.e., no molecules of the original substance remain. Needless to say, that fact engenders great consternation if one postulates a biochemical modus operandi. However, recent research by British electrical engineer Cyril Smith has shown that water, when exposed to electromagnetic frequencies and then succussed, retains the memory of ; those frequencies. Additionally, Japanese researcher Masaru Emoto has demonstrated with substantial photographic evidence that ice crystal morphology can be directed into reproducibly unique patterns by exposing the freezing water to particular pieces of music, or even prayers! Thus, the notion that a substance can leave an energetic imprint on water, which can be strengthened with successive energetic inputs percussive force ; , and increased concentration of the water i.e., decreased solute concentration ; begins to appear less preposterous. And, since we are about 55-70% water, the concept that energetically imprinted water could affect us if ingested may not be wholly implausible. The data from controlled trials suggest that homeopathy may be useful in the treatment of chronic pain. However, despite the ease with which remedies may be administered as indistinguishable pills, allowing for placebo control, there are methodological problems in designing standardized trials. Homeopathic remedies for complex problems are uniquely selected, such that ten patients diagnosed with trigeminal neuralgia could easily receive ten different remedies. Hence, practitioner skill in remedy selection is key, rendering truly relevant clinical studies difficult to design. I use homeopathy extensively in my practice, and have found limited success in treating chronic pain patients with this modality, principally in patients with facial neuralgia. I consider this to be as much a testament to my inadequate skills as a homeopath, as to an inherent failure of the technique. For, I have enjoyed startling triumphs using homeopathy in the treatment of other less complex conditions, such as: marked 1 3 ; reduction in fracture healing time; prompt 2 weeks ; healing of chronic non-union.

For each person with suspected or confirmed active TB disease, a specific clientcentered, case management and treatment plan emphasizing adherence to therapy, should be developed that includes types of drugs to be prescribed, anticipated changes to and duration of therapy, methods of assessing and ensuring adherence including directly observed therapy DOT ; , methods of monitoring for adverse drug reactions, and necessary evaluations with appropriate time frames. Each person with suspected or confirmed active TB disease should be educated and counseled about the disease and the importance of anti-TB medications and their potential side effects and adverse reactions. All persons should be educated about the need for client adherence to prescribed anti-TB therapy and medical follow-up.

Although this established strategy provides useful results about the relative selectivity of an inhibitor, it has several shortcomings since only a small subset of the more than 500 human protein kinases can be tested and alternative protein targets such as different types of enzymes are usually excluded altogether 17 ; . A conceptually different approach to study inhibitor function employs suitable small molecule derivatives, which can be covalently immobilized on chromatography beads and then used for the affinity purification of cellular target proteins 18-21 ; . In combination with sensitive mass spectrometry MS ; analysis, this chemical proteomics strategy has the potential to reveal the cellular target proteins of small molecule inhibitors in extracts from cultured cells, tissues or even whole organisms. The successful implementation of chemical proteomic techniques critically depends on the efficiency and performance of the affinity purification procedures. We have recently reported an affinity chromatography technique, which operates under optimized biochemical conditions and allowed the identification of several previously unknown cellular targets of the p38 inhibitor SB 203580 and bisindolylmaleimide-type protein kinase C inhibitors 22-24 ; . Here, we have adapted this proteomic approach for a pyrido[2, 3-d]pyrimidine derivative and identified more than 30 new cellular protein kinase targets of this inhibitor class. At the same time, these remain largely voluntary industry positions. They are not comprehensive and integrated policies, they are not consistent, and they lack legal force. The Hope for a Technology Solution Somewhat by coincidence, inexpensive radio frequency identification devices RFID ; and a bold businessbased standards approach, the Electronic Product Code EPC ; system, emerged from their research home at MIT over the past few years51. By attaching small, inexpensive RFID tags, programmed with individual EPC identity numbers, and placing web-connected readers in the warehouses of all supply channel members, this system could offer the promise of full visibility of the movement of individual units of drugs from manufacturer to dispenser. If 25OHD levels do not respond adequately to increased doses of oral ergocalciferol, UVB therapy Gronowitz et al, 2003 ; , or more polar vitamin D analogues such as calcitriol or alfacalcisol see Table 8.1 ; should be considered [C]. Upon questioning, the patient recalled a history of forearm fracture and pelvic fracture several years before his presentation but denied any height loss. When he presented to our clinic, there was no bone mass or tenderness on physical examination of the anterior or posterior ribs or thoracolumbar spine on palpation. He was on Tritace ramipril ; , Zyloric allopurinol ; , and Esidrex hydrochlorothiazide ; . Laboratory studies available at that time included blood urea nitrogen 74 mg dl and serum creatinine 1.8 mg dl. His calculated creatinine clearance based on 24-h urine collection was 32 ml min. Serum calcium was 8.9 mg dl with normal serum albumin level, serum phosphorus 2.8 mg dl, and alkaline phosphatase 317 IU liter. Urinary calcium excretion was 20 mg d. Intact PTH was 666 pg ml and serum 1, 25 OH ; vitamin D was 27 pg ml. Additional laboratory results included a serum 25 OHD of 25 ng 800 IU d vitamin D ; , normal serum and urine protein electrophoresis, serum testosterone levels, TSH, serum glutamic-oxaloacetic transaminase, and serum glutamate pyruvate transaminase. Antiendomysial antibodies were negative. Ultrasound of kidneys showed a small right kidney with presence of simple cortical cysts in both kidneys. BMD Z-score was 4.1 at the lumbar spine, 1.7 at the total hip, 1.0 at the femoral neck, and 4.3 at the 1 3 radius. Instant vertebral morphometric assessment by dual-energy x-ray absorptiometry from T5 to L5 revealed no evidence of vertebral compression fracture. The working diagnosis was severe secondary hyperparathyroidism with severe bone loss and osteitis fibrosa cystica in a 55-yr-old male patient with stage 3 chronic kidney disease. The patient was originally started on alcacalcidol 0.25 g d along with calcium carbonate 1000 mg d and 800 IU of vitamin D3. The alfacalcidol was subsequently increased to 0.5 g daily. Within 4 months of therapy, his serum calcium was 9.5 mg dl, serum phosphorus was 2.8 mg dl, and his PTH level had dropped to 389 pg ml. The dose of alfacalcidol was again increased to 1 g d, which resulted in further decline in the PTH level to 125 pg ml at months of therapy. Ten months after starting vitamin D therapy, BMD had increased by 12% at the spine and 18% at the hip with normalization of Z-score at that site, but there were no changes at the forearm. CT scan of the chest revealed marked regression in the lytic lesions in the ribs and dorsal vertebrae with a decrease in the size of all lesions and filling in of bone by sclerosis at 10 months of therapy Figs. 1B, 2B, and 3B ; . At that time, weekly alendronate was added at a modified dose of 35 mg wk half-tablet of the 70-mg preparation ; due to the renal failure. Because the calcium level had increased to 10 mg dl, the dose of alfacalcidol was cut down to 0.5 g d, which resulted in a rapid increase of the PTH level to 203 pg ml after 3 months of decreasing.
